Any injury or incident that required first aid treatment during training or a match an accident form needs to be sent to Paul Jeyes and Paul Banner within 24hrs please.
Any injury or incident that required hospital attendance Paul Jeyes and Paul Banner need to be notified on the same day and the accident form needs to follow again within 24hrs please.
If an injury or incident requires ADMISSION to hospital then Paul Jeyes and Paul Banner need notifying on the same day and need both our accident form and the serious injury reporting form completing and sending to myself and Banner.
Please see the serious injury protocols for your information.
Paul Jeyes or Paul Banner will then pick this up and adhere to the process so it is imperative that we get accurate and detailed information.
